Investigating and Confirming AI PSIF Nominations

Once a description has been analyzed and receives a strength score of 3 or higher, investigators can decide themselves if the incident or near miss is a Potential Serious Injury or Fatality (PSIF) or have the option to use the AI PSIF Insights tool. This evaluation is supported by the platform’s AI, which provides reasoning for its nomination recommendation.

Accessing the AI PSIF Insights Tool

The PSIF evaluation tool becomes available to users with edit permissions on Incident and Near Miss records once:

• • The “Is this a serious injury or fatality?” question must be answered with no
  • The Description Analyzer has been used, and
  • The score is 3 or higher

At this point, the Ask Velo button will appear in the Potential Serious Injury or Fatality section of the investigation page.

Viewing AI PSIF Insights Recommendations

1. Open the relevant Incident or Near Miss record.
2. Ensure the Description Analyzer has already been used.
3. Select Ask Velo button in the Potential Serious Injury or Fatality section.
4. The AI will return:
   • An of whether the record is a PSIF or not a PSIF
   • AI-generated reasoning behind the evaluation

You may then choose to accept or reject the recommendation.

Accepting or Rejecting the Evaluation

AI PSIF Insights recommends the record as a PSIF

• • Accepting confirms the record as a PSIF
    

• • Rejecting confirms the record is not a PSIF and requires a short rejection reason (e.g., AI lacked context, PPE prevented risk, etc.)

AI PSIF Insights recommends the record as a non-PSIF

• • Accepting confirms the record is not a PSIF.
    

• • Rejecting confirms the record is a PSIF but requires a short explanation why you are overriding the AI evaluation (e.g., AI lacked context from additional investigation, PPE prevented risk, etc.)

Once submitted, the decision is logged and may trigger a notification email to designated stakeholders.

The record can be closed without making a PSIF decision. If left undecided, the nomination status will remain Pending.

What Happens When You Edit a Record

If the description is edited after the evaluation:

• • All AI-generated artifacts (such as strength score, suggestions, AI PSIF evaluation, and reasoning) are cleared
  • The platform will prompt you to re-select Ask Velo in the Description first, followed by re-selecting Ask Velo in the PSIF section for a new evaluation before saving the record

This ensures that the nomination is always based on the most current information.
