This article outlines the steps involved in managing Hazard Identification and identifies who is responsible for each step. Hazard Identification refers to a condition that has the potential to result in a negative outcome.
Note: Before adding your first Hazard Identification record to the platform, ensure the role Default Responsible for Hazard Review is assigned to at least Level 1 of the location tree. Without this, the record cannot progress through the workflow.

Draft (Reported By)
A Hazard Identification can be initiated by any user on the platform. This step captures the who, what, when, and where of the identified hazard, as well as the associated risks and potential outcomes.
Key activities during this stage:
- Inputting initial hazard details.
- Conducting a risk analysis.
- Creating associated action items for corrective or preventative measures.
Key points:
- Action items can remain open even if the Hazard Identification record has been closed.
- The record cannot progress if the Default Responsible for Hazard Review role is not assigned.
Review (Responsible for Hazard Review)
During this step, the person responsible for reviewing the Hazard Identification verifies the accuracy of the recorded details, assesses the proposed corrective actions, and ensures the risk analysis is thorough.
Tasks during this step include:
- Reviewing the captured hazard details.
- Confirming the appropriateness of the corrective or preventative action items.
- Adding any additional notes or updates, as needed.
Closed (Responsible for Closure)
Once all hazard details have been reviewed and finalized, the Hazard Identification can be closed.
Key points:
- Closing the Hazard Identification does not require all action items to be completed. Open action items will continue to be tracked independently.
- Closed Hazard Identifications will remain accessible in the Hazard Identification Records for future reference.
