Reporting an Incident

Reporting an incident allows you to:

• Track the events that have occurred at your organization
• Perform due diligence through investigation of these events
• Ensure a similar incident does not occur or is less likely to occur as a result of your efforts

Everyone is encouraged to report an incident as quickly as possible after it occurs.

To report an incident:

1. Select Incident from the Add New menu.
   
   
2. Include all relevant information about the incident to ensure it is accurate.
   
   
3. Click the Submit First Report button to complete the Incident report.
   
   

Now that you’ve reported the incident, the person responsible for completing the next stage of work will be made aware of their task, and your job as the reporter is done!
