Your Incident Management account includes Agency Reports which can generate OSHA 300, OSHA 300A, and OSHA 301 reports. As the Occupational Safety and Health Administration (OSHA) is the regulatory authority for the United States, it is not necessary to generate these reports for locations outside of the United States.
- OSHA 300 provides a log of work-related injuries and illnesses and all required information. Incident details are pulled from the Incident Management system, and establishment information (e.g., city) is pulled from the location profile managed by your System Administrator. This report is intended to be printed on legal (8.5×14) paper for posting and OSHA record-keeping purposes.
- OSHA 300A provides a summary of work-related injuries and illnesses. Total numbers of cases and days are pulled from the Incident Management system, the number of employees and total hours worked are pulled from labor information within the Metrics module, and establishment information (e.g., city, SIC) is pulled from the location profile managed by your System Administrator. This report is intended to be printed on legal (8.5×14) paper, signed by a company executive, posted, and kept for OSHA record-keeping purposes.
- OSHA 301 provides a completed OSHA 301 form for each recordable incident at the selected location. This report is intended to be printed on standard (8.5×11) paper.
- OSHA 300A Electronic Submission (CSV) is used to generate the OSHA 300A in a CSV (Comma Separated Value) format intended for uploading to OSHA.
How to Generate Reports
Access
To access the Agency Reports, select Monitor > View All Reports.
Then select Agency Reports from the menu options on the left sidebar (also known as the index).
Select the report you would like to run from the reports available. You can review the report’s parameters to ensure it meets your reporting requirements.

Note
The default parameters are set to what OSHA expects to receive, but you are responsible for confirming that the parameters are correct before generating and submitting any reports.
Generate and Export
Once you have set your parameters, determine how to generate the report if there are multiple locations. It is possible to generate a single report, a report for each selected location, or a report for each applicable location as a level of the location tree.
Note
OSHA expects separate reports for each “establishment” that is anticipated to be in operation for one year or longer (see 29 CFR 1904.30).
Next, select the Run Report button in the top right corner.
- A PDF file will be generated for the OSHA 300, OSHA 300A, and OSHA 301 reports.
- A CSV file will be generated if you are running the OSHA 300A Electronic Submission.
Setting the Establishment Name
The system generates the Establishment Name on the reports according to the following rules:
- If a single level-1 or level-2 location is selected, that location name is used as the establishment name.
- If two or more locations are selected, their lowest common parent is used for the establishment name.
- If one or more locations below level 2 are selected, the establishment name is prefixed with the name of the lowest common parent location at level 2 or above.
- If the children locations are in different level-2 or level-3 branches (so that their lowest common parent is at level 2 or level 1) the children’s names are omitted, and only the level-2 or level-1 name is shown.
