Reporting a Near Miss

Reporting a near miss allows you to:

• Track the events that could have resulted in incidents in your organization
• Perform due diligence through investigation of these events
• Ensure an incident does not occur or is less likely to occur as a result of your efforts

Everyone is encouraged to report a near miss as quickly as possible after it occurs.

To report a near miss:

1. Select Near Miss from the Add New menu.
   
   
2. Include all relevant information about the near miss to ensure it is as accurate as possible.
   
   
3. Click the Submit First Report button to complete the Near Miss report.
   
   

Now that you’ve reported the near miss, the person responsible for completing the next stage of work will be made aware of their task, and your job as the reporter is done!
