AI Root Cause Identifier
The AI Root Cause Identifier compares the incident description, event details, and selected hazards against your Root Cause list to recommend the most relevant items. If you use a configured Root Cause list instead of the standard one, the feature may also highlight additional VelocityEHS Root Causes that aren’t currently in your list so you can consider adding them.
What It Solves
- Root causes are often identified differently across teams, making it harder to agree on prevention strategies.
- By suggesting root causes at the most specific level available, the AI Root Cause Identifier helps investigators align on findings that are more actionable.
- This improves prevention efforts and creates more consistency across investigations.
Requirements
- The incident description must score 3 or higher with the Description Analyzer.
- You must be able to edit and change the recommendations where you are using Vēlo.
- You need to select the Ask Vēlo button to generate root cause recommendations.
Note
Adding Event Details and Hazards can improve the accuracy of root cause recommendations, but Hazards are not required to generate them.
Using the Root Cause Identifier
- Enter or update the incident description. Add event details and hazards for better results.
- Select Ask Vēlo in the Root Cause section to generate recommendations.
- Review the recommendations. Root causes are suggested to the most detailed level available in the Root Cause tree.
- Select or deselect root causes as needed.
- Save your selections.
After saving, Vēlo-suggested and manually added root causes appear together as part of the investigation.
Key Behaviors
- If the incident description score is below 3, root cause are not available. The platform displays:
“The description must be ≥3 to use this AI feature.” - If the description or event details are updated, Vēlo prompts with:

“You’ve made updates to the following:
- Description
- Event Details
- Hazard Types
To get the most accurate AI analysis, click ‘Ask Vēlo’.”
- Once root causes are saved, manual and Vēlo-recommended root causes appear together in the Root Cause section.
- The Audit Trail records which root causes were saved, including those suggested by Vēlo that are missing from your Root Cause list.
