AI Corrective Action Advisor

The AI Corrective Action Advisor maps corrective actions to the root causes you select. It recommends pre-populated actions (title and description) aligned with those root causes. This helps you quickly assign and track meaningful follow-ups that address the underlying issues.

What It Solves

• • Corrective actions are often vague or incomplete, leading to poor follow-through.
  • By suggesting targeted actions for each root cause, the AI Corrective Action Advisor supports more consistent and enforceable solutions.
  • This increases accountability and strengthens risk reduction efforts.

Requirements

• • The incident description must score 3 or higher in the Description Analyzer.
  • At least one root cause must be selected before corrective actions can be generated although you can manually create corrective actions at any time.
  • You must be able to edit and change the recommendations where you are using Vēlo.
  • You need to select the Ask Vēlo button to generate corrective action recommendations.
  • You must be able to edit and change the recommendations where you are using Vēlo.

Using the Corrective Action Advisor

1. 1. Enter or update the incident description and root causes. Include event details and hazards if they are available.
   2. Select Ask Vēlo in the Corrective Action section to generate recommendations.
      
   3. Review the recommendations. Each root cause receives one or more corrective actions, with pre-populated title and description.
   4. Modify, add, or delete corrective actions as needed.
   5. Assign a responsible person and due date for each corrective action.
   6. You can manually add additional corrective actions at any time.
   7. Save your actions.

After saving, Vēlo-suggested and manually added corrective actions appear together as part of the investigation.

Key Behaviors

• • If the incident description score is below 3, corrective action recommendations are not available. The platform displays:
    “The description must be ≥3 to use this AI feature.”
  • If the description or event details are updated, Vēlo prompts with:

“You’ve made updates to the following:

• • • Description
    • Event Details
    • Hazard Types
    • Root Causes

To get the most accurate AI analysis, click ‘Ask Vēlo’.”

• • Each root cause must have at least one corrective action to progress the workflow.
  • Every corrective action must be completed before the incident record can be closed.
  • A responsible person and due date are required for each corrective action before saving.
  • Once corrective actions are saved, manual and Vēlo-recommended items appear together in the Corrective Action section.
  • The Audit Trail records which corrective actions were saved, including those suggested by Vēlo.
