This article outlines the steps involved in managing an incident and identifies who is responsible for each step. An incident is an unplanned event that results in a negative outcome.
Important: Before adding your first incident record to the platform, ensure the Default Responsible for Incident Investigation role is assigned to at least Level 1 of the location tree. Without this, the record will not progress in the workflow.

Draft (Reported By)
The first report of an incident can be entered by any platform user. This step captures the basic details of the event: who, what, when, and where. All items in this report can be edited in later stages.
Key Points:
- The record cannot progress if the Default Responsible for Incident Investigation role is not assigned.
Investigation (Responsible for Investigation)
At this stage, the assigned investigator updates the initial report and completes the investigation. This step involves documenting key details, including recordable/reportable information, costs associated with the incident, root causes, and action items.
Key Features:
- If an injury or illness is reported, an Agency Report (OSHA 301 Form) can be generated and printed.
- Each root cause must be linked to at least one corrective action.
Review (Responsible for Investigation Review)
The Review status serves as the final check. The reviewer ensures all information is accurate, confirms the appropriateness of action items, and completes any additional documentation.
Pending Closure (Responsible for Closure)
This status acts as a “holding area” while all assigned corrective actions are completed. If lost or restricted work is noted, the record must remain in this status until the employee returns to full capacity or the work restriction period ends.
Notes:
- There is no time limit for how long a record can remain in this status.
- Incidents cannot be closed while action items remain open.
- Incidents cannot be closed while restricted and lost time is open
