July 2025
Application Release Version 11.68.0, 7/17/25
Enhancements:
- The Risk Count Matrix webpart now supports department/sub-area filtering.
- The Location Trend webpart was updated to reflect recent JSA improvements.
- The Attachments tab (in risk worksheets, JSAs, etc.) now shows the date an attachment was added and who added it.
- JSA Properties now display associated step-level hazard actions.
Application Release Version 11.67.1, 7/3/25
Enhancements:
- Updated the site bowtie review step to display either Control Name or Description depending on system configuration
- Restored the 100-row display option in risk worksheets
- Multiple hazards can now be assigned to a single JSA step
Bug Fixes:
- Resolved an issue that prevented users from deleting or moving sites.
- Resolved an issue that caused an error when double-clicking the Base Controls field in a job safety analysis worksheet.
- Resolved an issue that prevented the display of comments added to individual line items in a Job Safety Analysis.
- Resolved an issue that occurred when duplicating a JSA step with attached hazards; this action could unintentionally modify the hazards in the original step.
June 2025
Application Release Version 11.66.0, 6/19/25
Enhancements:
- Refreshed translations across all supported languages to improve clarity and consistency for global users.
- The Risk Verification Attachments tab now shows all files added during a verification or assurance activity, including photos and documents uploaded via the mobile app.
- Users can now open dashboard items in a new tab or window using standard browser actions (right-click or Ctrl+click), making it easier to view multiple records at once.
Bug Fixes:
- Resolved an issue that occurred when adding LOPA entries into a HAZOP with LOPA risk worksheet; newly added LOPA risk issues, causes, and consequences now automatically sync with the HAZOP sheet.
- Resolved an issue that caused an error when creating a new perspective on the Bowtie page if the selected language for the application was not English.
- Resolved an issue that was preventing user access to the Admin > Terminology page when the selected language for the application was not English.

Risk Assessments:
- Bowtie status colors now accurately reflect colors selected in the Configuration Editor.

- Fixed a bug that was preventing the display of users for selection from the Verifier drop-down field in the Submit for Approval window.
- Users with “read-only” access to bowties can no longer delete or edit bowties. These options are now correctly hidden based on permission settings.

Bug fixes:
- Fixed an issue that was preventing data from uploading successfully.
- A bug that caused duplicated issues to incorrectly display two causes and two consequences has been resolved. Duplicated risk issues will now correctly show a single cause and two consequences.
- A bug preventing users from opening master bowties has been fixed.
- We’ve corrected the alignment of the Top 10 Global Items webpart on the dashboard to prevent overlapping visuals.
- The Control Verification Templates Generated Sets page now correctly displays all expected data, consistent with the default view.

- Risk study APIs have been optimized, leading to improved performance and stability when loading worksheets.
- A security improvement has been made to help detect malicious files in the upload/attachment functionality.
- The Verification Activity Timeline dashboard webpart filter designs have been updated to be consistent with the rest of the system.
- The Top 10 Global Items dashboard webpart now displays additional contextual data when drilling down into JSA control data.

Application Release Version 11.50.0, Oct. 25, 2024
- Upgraded the Operational Risk database infrastructure to provide more consistent and faster performance.
- Added an Approval History tab to worksheet properties to track formal approvals and rejections.
- Added a Comments field to provide a place to record notes when rejecting a risk study.
- For Operational Risk accounts in which the Auto Evaluate Mitigated Likelihood feature for worksheets is active (under Admin > Configuration Editor > Risk Assessment), the Mitigated Risk column now displays the current mitigated risk from the base control calculation, as well as the risk matrix equivalent value it showed previously. This change helps users identify risks that are close to being reduced to a lower risk matrix value and provides context around how helpful the base controls being used are.

- A new filter button, located next to the existing Filter field on the Job Safety Analysis page, enables users to add JSAs from sub-locations to the JSA list to simplify searching and analytics across the location tree. As a result, a Location column displays and can be sorted.
- Top 10 Global Items dashboard webpart for JSAs now displays JSA title and task information in the drill-down data.
- The type-ahead Search bar in worksheets now searches inner table data (causes, consequences, controls, and actions) as it does with issue-level data.

Our second September release includes improved “insert new” functionality when using filters/search within tables and worksheets. Inserting rows and entities while filters are active will display the inserted items so that they’re immediately visible in the current view.
Application Release Version 11.47, Sept. 13, 2024
The first September release includes user support for the JSA list import/export, several styling changes, and some workflow optimization for verifications.
- The left navigation panel has been given a refresh to provide more screen space for worksheets/JSA/bowties and more intuitive navigation.
- Users can now initiate verifications in the desktop application from the Verification properties page, improving user workflow when arriving at a verification from a reminder email or from an action source link.

Application Release Version 11.46, Aug. 30, 2024
Our August release includes performance enhancements and user interface updates to the modal, worksheet, and button designs, as well as the Action page.
- Action Source information has been grouped at the bottom of the Action Primary Details page and users can now directly access the source record (worksheet, bowtie, or JSA) from this section.
- Bowtie and master bowtie table loading/filtering performance has been improved for faster navigation through assessments.
July 2024
Application Release Version 11.42, July 5, 2024
Our first July release includes the following updates:
- Risk assessment review participants and team leaders will now receive an email notification when a review is sent to “In Progress.”
- Users can now select multiple records (on the Worksheets, Actions, Bowties, and other pages in list/table format) and run a report, for example a Risk Study Report, from the Reports dropdown option at the top of the page, for only those selected records.

Application Release Version 11.41, June 21, 2024
Our second June release includes the following updates:
- The software now prohibits editing of bowties for inactive locations.
- A new column, Control and Influence, is available (Configuration Editor > Register Policies) to include in risk assessment worksheets to further categorize risks.
- Two new Recurring options have been added for verification templates: every 6 months and every 18 months.
Application Release Version 11.40, June 7, 2024
Our first June release includes the following updates:
- Filters were adjusted to contain an Exclude option to allow for greater control when viewing data.
- The Insert Copy action in worksheets will now assign a unique code to the inserted copy. This change will help users identify new copies and ensure the original worksheet is not inadvertently modified after copying.

Application Release Version 11.39, May 24, 2024
Our second May release includes the following updates:
- The software will now prohibit users from creating new risk assessment worksheets or editing existing worksheets for inactive locations. Some user interface elements were added to make this clear.
- Users can now batch-upload JSAs to create a large number of (blank) JSAs with coordinator, reviewer, due dates, and titles all predetermined.
- Users can now receive reminder emails for risk assessment worksheets approaching their end date and JSAs approaching their re-review date. No more expired risk studies or JSA reviews falling through the cracks!
Application Release Version 11.38, May 10, 2024
Our first May release includes some fixes for customer reported issues, updates to automated worksheet functions, and Phase 1 of a new batch feature for JSA.
- If a customer is using the Auto Evaluate Mitigated Likelihood setting (Configuration Editor > Register Policies), a Control Status score of “0” will now disable that base control. There is also new Configuration Editor setting (Update Proposed Values To Initial On Change) for automatic change of the proposed/future severity and likelihood on change of the initial (similar to the setting for residual).
- The filters for multiple Subareas and Departments required some updates after the change in cardinality. Those filters are back online and available.

Application Release Version 11.37.1, April 26, 2024
This release includes fixes for customer-reported issues, updates to department and subarea relationships to risk assessments, and a new view-only dashboard permission.
- An issue causing the Mobile App to not load all users when assigning an action or verification has been fixed.
- The simple table sorting and filtering has been updated to allow for multiple departments, subareas, and assets associated with worksheets, bowties, and JSAs.
- Phase 2 of an update to allow for multi-select of departments and subareas linked to worksheets, bowties, and JSAs has been completed.
- A new permission has been created to allow users to view (and select) all global dashboards, without allowing creation or editing of the global dashboard properties.

This release includes fixes for customer-reported issues, continued performance updates, and additional dashboard optimizations.
- Conditionally triggered recalculations for verifications and bowties will help improve performance.
- The Risk Count Matrix now loads summary data upon login and dashboard loading, greatly improving performance. Detailed drill-down data will be loaded when required and has been optimized to improve overall performance.
- Work is underway on an update to allow for multi-selection of Departments and Subareas linked to worksheets, bowties, and JSAs.

Application Release Version 11.35.0, March 29, 2024
This release includes bug fixes and some dashboard updates.
- Duplicates of bowties, risk worksheets, and JSAs now use the original code with an amended “revision number” for easier tracking of modified/copied studies for easier revision tracking.
- Selecting an entity in the Top 10 Global Items dashboard widget now takes users directly to the risk assessment/bowtie related to the entity.
- Phase 1 of the enhancements to how the Risk Count Matrix loads data is complete, resulting in quicker dashboard loading and a better experience for top-level data roll-up.

Our March release includes some major fixes, updates to system recalculations to improve overall performance, and a few quality-of-life enhancements.
- Conditionally triggered recalculations for global entities, actions, and risk assessment worksheets will help improve performance.
- The Insert New Item button is now always visible in risk assessment worksheets. No more scrolling!
- Risk issue Max risk rating values are now read-only when the system is set to auto-calculate.
- The risk verification issue that was causing users to be timed out or rate-limited has been resolved.
- The Dashboard now includes a new JSA Burn Down chart showing the number of Job Safety Analysis records due over time (Target) and the number completed (Actual).

Application release version 11.32.0
The February release includes the new Job Safety Analysis (JSA) function, an entry-level risk assessment tool within the Essential Operational Risk solution. JSAs help organizations identify and mitigate potential hazards associated with job tasks, driving operational efficiency, enhancing risk maturity, and ensuring overall workplace safety. JSA includes the following features:
- Simple assessment template for consistent data collection
- Initial risk calculation using up to four variables to enhance risk decision-making and granularity
- Automatic risk calculation using base controls to help non-experts evaluate risk levels expertly
- Risk-based analytics and dashboards to manage action items and hazard and control trends

Application release version 11.33.0
- The Top 10 Global Items dashboard item now includes a list of the specific entities contributing to the top 10. Additional content updates and deeper drill-down capability coming soon!
- The backend recalculation services responsible for administration configuration have been optimized to improve performance across the board.
- Hovering over the drop-down base control list in risk assessment worksheets and JSAs now displays additional properties of the selected base control to help users understand control selection and see additional context.
- The Insert Copy function in a JSA now searches the current column for entities within the JSA, streamlining the process of transferring data from an existing JSA to a new one. This saves time and ensures that the most accurate information is used.
- Keyboard navigation functionality was added for drop-down menus in risk assessment worksheets and JSAs.

The December release includes the following enhancements:
- “Sticky” filters and pagination: The system will now retain all changes applied by each user to filters, searches, and number of entries per page selections.
- Master bowtie deletion: Master bowties can now only be deleted in “draft” status.

The Location Trends dashboard item now summarizes risk levels from risk worksheets to help users track them over time and to show management how much risk is being reduced.

Application release version 11.25.0
The October release includes a series of enhancements, all with the goal of enhancing the overall user experience:
- Side navigation menu: Minor changes were made to the side navigation menu. As Operational Risk prepares to integrate with the Accelerate Platform in 2024, expect additional small user interface changes to align with other products on the platform.
- Top 10 Global Items filters: A new risk matrix filter was added to the Top 10 Global Items Dashboard item. Use this filter to focus on the highest risk items across worksheets and bowties.
- Duplicating verification templates: Users can now copy verification templates to other locations. Share your favorite templates/schedules with your colleagues.
- Site status for bowtie controls: The site-specific status of a master/site bowtie control is now a visible field within site bowties to help managers track control differences site to site.

Application release version 11.23.0
The September release includes enhancements aimed at elevating the overall user experience:
- Terminology Editor additions: The Terminology Editor now include Controls (as an option for Combined Scenario for simple assessments), Major Accident Hazard (as an option for Critical Risk, Principal Hazard, etc.), and Risk Score (as an option for Risk, Risk Rating, etc.) for those gamifying their risk management efforts.
- Risk Count Matrix: Users can drill down in the risk count matrix from the Dashboard to go directly to the risks within the worksheet or bowtie, enabling quicker assessment updates, review, and communication.
- Notifications: Email notifications for risk worksheet workflows (assignment and review) were enabled.
August 2023
Application release version 11.20.0
The August release includes performance improvements and user experience updates:
- Worksheet issues, Max Risk: The logic to determine which consequence is pulled into the “Max Risk” columns for each issue has been updated to reflect the prioritization score for the consequence categories (Administration > Configuration Editor). When two or more consequences under the same issue have the same risk, the most important category is pulled as the Max Risk value.
- Worksheet print: New Print Preview and Print (from browser) options were added to the risk assessment worksheets.
- Worksheet Insert Link, Insert Copy: The text shown when inserting a link or copying will now match the column information where the inserting or copying is occurring.
July 2023
Several bugs have been fixed for the July update, many centering around performance issues, bowtie calculations and verifications. Some other items to highlight are below.
- Worksheet enhancements:
- Worksheet approver. There is now an indicator to show who is currently reviewing a worksheet for easier tracking of where a worksheet is in the review process.
- Print your worksheets. We’ve added a worksheet print preview function so you can use the browser print to print the worksheet in the in-app format. This ensures that your perspective and the visual styling of a worksheet is preserved in a simple-to-read PDF or printed document.
- Consequence categories. We’ve added functionality to enable prioritization of consequence categories depending on your organizational risk profile (for example, prioritizing health and safety risks over financial).
- Navigation improvements. We’ve added page titles across the system to create better bookmarks and smoother system navigation. Using the Back button in your browser will now show a list of each page you’ve visited.
- Bowtie enhancements. As well as squashing a few bugs in bowties, we’ve removed the “operation in progress” and “operation complete” messages that appeared when editing a bowtie.
- Insert copy/insert link. Using the insert copy/insert link function in a worksheet now pulls the picklist from the column selected. No more blank options!
- Mobile app. We resolved an issue that occurred when trying to open the mobile app offline; all functionality has now been restored.
June 2023
We’ve spent the month of June squashing bugs, adding stability, and increasing speed across all areas, with continued improvements to almost every aspect of bowties. The following behind-the-scenes changes also occurred:
- Dashboard filtering. The Risk Count Matrix can now be filtered by assessment status, making it easier to view issues from assessments that are completed or under review. Additional improvements coming in July (v.11.17)!
- Better bowties. Saved bowtie layouts are back! Node positions will persist when reopening a bowtie and will not reset on a refresh. This reorders the nodes on reports as well.
- Faster bowties. Bowtie performance, including movement and modification of controls, has significantly improved.
- Global or local? A new globe icon identifies entities that apply throughout the system and are not associated with a particular location, for example, base controls and performance standards. When in doubt, click on the icon.
- Added redundancy. Verifications in progress are no longer locked to the current assignee. If someone is on leave, or otherwise preoccupied, this change ensures that verifications can still be completed on time. All it takes is a simple swap in the verification details!
- Context matters. Action emails now contain additional detail and context; the Action’s code, risk location, and linked entity information are now included in the standard notification email template.

Accelerate Risk is here!
Operational Risk has undergone a major upgrade with significant improvements to the user interface and page load times. Several functional enhancements have been made in response to you, our users, and customers.
