December 2025

Application Release Version 12.7.0, 12/18/25

• Improved loading time when performing risk verification.
• Implemented a new workflow for editing worksheet perspectives. This enhancement introduces additional steps when saving changes to perspectives, which will reduce the risk of unintentional modifications.

November 2025

Application Release Version 12.5.0, 11/20/25

• Updated bowtie import function to include escalation control during the import process. 
• Resolved issue that was causing tags and base controls to not be linked during a worksheet import. 
• Resolved issue that caused bowtie layout to be lost when switching browser tabs. 
• Resolved issue that caused a new, blank Job Safety Analysis hazard row to be added when accepting or saving hazards, after using the (Vēlo) Hazard Analyzer.

Application Release Version 12.4.0, 11/6/25

• The JSA step workflow now allows single selection of MAE tags. 
• Users can now unlink site bowties from a master bowtie; prior to unlinking, the application displays affected locations for greater transparency. 

October 2025

Application Release Version 12.3.0, 10/23/25

Enhancements:

• Change tracking: Introduced batching support for bulk operations, such as bowtie imports, to improve efficiency and reliability
• Enabled automatic recalculation of risk worksheets upon duplication to ensure data accuracy
• Users can now print up to 100 bowties at once (previously limited to ~25 due to browser constraints)

Enhancements 

• Ask Vēlo: AI-powered Hazard Analyzer & Control Recommendations functionality now available, including three new AI report options
• Performance optimization: Improved type-ahead response time
• Worksheet archiving: Archive function is now available for Draft and In-Progress worksheets
• Department/sub-area merge: Introduced merge functionality for department and sub-area management

September 2025

Application Release Version 12.1.0, 9/11/25

Enhancements 

• The JSA Burn Down webpart on the Dashboard now accounts for JSA re-review dates 

• The To-Do list now includes all upcoming JSA re-reviews 

Bug Fixes  

• Fixed an issue causing duplication of sub-areas when importing risk worksheets from Excel 

• Fixed an issue that prevented full display of available selections when a dropdown field was located at the bottom of the page 

• Fixed an issue in JSA worksheets that allowed Groups to appear for selection in the Implementer (or Assigned To, Action Owner, Responsible, etc.) field when Configuration Editor > Action Responsible Role was set to Person 

• Fixed missing translations across the system

Application Release Version 11.71.0, 8/28/25

This release focused on stability and quality improvements with a strong emphasis on bug fixes. The following were resolved:

• Location Trend discrepancy in reporting
• Type-ahead indexation timing out
• Issue with importing existing dashboard pages
• JSA duplication issue
• Bowtie Print Preview display issue
• Missing translations in multiple areas
• Issue with updating the Terminology Editor

Application Release Version 11.70.0

• Enhanced the Risk Scenario Details report to show actions linked to causes/consequences to improve traceability and context in bowtie analysis.
• Fixed an issue with attachments in Verifications.

July 2025

Application Release Version 11.69.0, 7/31/25

Enhancements:

• Made performance improvements to the Top 10 Global Items webpart on the dashboard
• Reference links added in the Attachments tab (available for most entities in the system) are now automatically converted to safe, active hyperlinks
• Added Reviews tab to JSA worksheets Properties and updated the To-Do list to reflect the review items
• Users can now select multiple hazards per task in a JSA worksheet, and multiple controls per hazard

Bug Fix: Resolved issue with duplicating JSA worksheets

Application Release Version 11.68.0, 7/17/25

Enhancements:

• The Risk Count Matrix webpart now supports department/sub-area filtering. 

• The Location Trend webpart was updated to reflect recent JSA improvements. 

• The Attachments tab (in risk worksheets, JSAs, etc.) now shows the date an attachment was added and who added it. 

• JSA Properties now display associated step-level hazard actions. 

Application Release Version 11.67.1, 7/3/25

Enhancements:

• Updated the site bowtie review step to display either Control Name or Description depending on system configuration
• Restored the 100-row display option in risk worksheets
• Multiple hazards can now be assigned to a single JSA step
• Removed bowtie history feature and replaced with the new and improved enterprise Change Log feature

Bug Fixes:

• Resolved an issue that prevented users from deleting or moving sites.
• Resolved an issue that caused an error when double-clicking the Base Controls field in a job safety analysis worksheet.
• Resolved an issue that prevented the display of comments added to individual line items in a Job Safety Analysis.
• Resolved an issue that occurred when duplicating a JSA step with attached hazards; this action could unintentionally modify the hazards in the original step.
• A location added by a Super Administrator will remain visible and will no longer be archived when Operational Risk is disabled for that location
• Resolved an issue that was causing an error when attempting to download an attachment from a control verification record

June 2025

Application Release Version 11.66.0, 6/19/25

Enhancements:

• Added Entity Type column to change log details. 
• The location Application selector (in Accelerate Admin Settings > Locations) now makes available/hides locations where Operational Risk is applied/deselected, respectively. 
• Refreshed translations across all supported languages to improve clarity and consistency for global users. 
• The Risk Verification Attachments tab now shows all files added during a verification or assurance activity, including photos and documents uploaded via the mobile app. 
• Users can now open dashboard items in a new tab or window using standard browser actions (right-click or Ctrl+click), making it easier to view multiple records at once. 

Bug Fixes:

• Resolved an issue that prevented users from downloading change logs. 
• Resolved an issue that prevented users from downloading attachments from the Risk Verification Attachments tab. 
• Resolved an issue that occurred when adding LOPA entries into a HAZOP with LOPA risk worksheet; newly added LOPA risk issues, causes, and consequences now automatically sync with the HAZOP sheet.
• Resolved an issue that caused an error when creating a new perspective on the Bowtie page if the selected language for the application was not English.   
• Resolved an issue that was preventing user access to the Admin > Terminology page when the selected language for the application was not English.  

Application Release Version 11.65.0, 6/5/25

Enhancements:

• Location Trends dashboard webpart now supports filtering by Consequence Category.
• The Attachments and References tabs have been consolidated into a single tab.

Bug Fixes:

• Fixed an issue with the paging option at the bottom of the Risk Assessment worksheet.
• Fixed an issue with the incorrect Risk Assessment worksheet tab (HAZOP vs. LOPA) displaying after importing LOPA data from Excel file.
• Fixed sorting issue with References on the Attachments tab.
• Fixed Control Verification Templates Generated Sets change tracking issue.
