Initiating an Emergency Change Request

An emergency change is intended to be put in place immediately in an emergency situation. This abbreviated process skips over the initial approval process to allow for quick action when necessary. An emergency change can also be made permanent, if needed.

To initiate an emergency change request:

1. From the navigation menu, select Operational Risk > Management of Change.
   
2. Select Add New at the top of the Change Requests page and select the change request category Emergency Change.
   
3. Identify the Emergency Approver, the individual who will be responsible for approving the emergency change request.
   
   Note: At any time during this process, you can select Options > Save Draft to save your work in the Draft stage, prior to submitting the request.
4. Indicate the Reason for Emergency from the selections.
5. Add a Title for the proposed change, a Description, and Justification for it. Describe its Impact on the environment, heath, and safety.
6. Select the Change Type(s) (for example, Process Technology, Procedure, Chemical, etc.). Change Types are defined upon system setup by an administrator in the EHS List Editor.
7. Select the location Where the proposed change would occur and what equipment would be involved in the change. Note that the location cannot be changed after you’ve submitted the request.
8. Indicate When the change would occur, including the initiation date, target completion date, and expiration date.
9. Identify the individual responsible for pre-startup review, the Implementer, and the MOC Coordinator.
10. If this request requires attachments, select the Attachments tab and add the relevant file(s) and/or web link(s).
11. Select Submit Change Request at the top of the page.
