Initiating a Permanent or Temporary Change Request

Initiating a permanent or temporary change request allows you to:

• • Reduce the risk of unintended consequences due to change
  • Seek and maintain records of technical and financial input to a change
  • Provide visibility to changes that are being planned or are already underway

Group meetings often occur to discuss changes before initiating them in the Management of Change software.

To initiate a permanent or temporary change request:

1. From the navigation menu, select Operational Risk > Management of Change. Select Add New at the top of the Change Requests page.
   
2. Select a change request category: Permanent Change or Temporary Change.
   
3. Add a Title for the proposed change, a Description, and Justification for it. Describe its Impact on the environment, heath, and safety. 
   Note: At any time during this process, you can select Options > Save Draft at the top of the page to save your work in the Draft stage, prior to submitting the request.
4. Select the Change Type(s) (for example, Process Technology, Procedure, Chemical, etc.). Change Types are defined upon system setup by an administrator in the EHS List Editor. 
5. Select the location Where the proposed change would occur and what equipment would be involved in the change. Note that the location cannot be changed after you’ve submitted the request.
6. Indicate When the change would occur, including the initiation and target completion dates.
7. Identify the MOC Coordinator, the person responsible for following up in the next stage of the change request.
8. If this request requires attachments, select the Attachments tab and add the relevant file(s) and/or web link(s).
9. Select Submit Change Request at the top of the page. The MOC Coordinator will be notified.
